Welcome to my office. I am pleased that you have chosen me to serve you at this time in your life. Please read the following information carefully and feel free to ask questions or discuss it with me during your first session.

 My practice uses a unique and personalized collaborative treatment model which includes incorporating loved ones, and consultations with medical providers, dietitians, schools, and coaches.

Appointments:

Appointments may be scheduled by speaking directly with me. An appointment typically lasts 50 minutes. If you are unable to keep an appointment, please give me 24 hours notice. Otherwise, unless you are physically sick, you will be charged $85.00 for the time reserved. If you miss more than two consecutive appointments without notifying me, you will be subject to termination as a client. 

I follow the code of ethics of the following boards and organizations:

-The South Carolina Board of Social Work Examiners
-South Carolina Society for Clinical Social Work
-National Association of Social Work

Fees:

Fees will be discussed in your first session. Due to the individualized and specialized approach used in this practice, the fee structure is as follows: there is a $175.00 initial fee for the education/coaching/orientation to our unique and individualized treatment plan (insurance does not cover this fee.) The fee for individual, couple, and family sessions is $120.00 per 50-minute session.  You will be notified in advance. Some sessions may last more than 50 minutes. The usual rate will be charged, proportionate to the time used. 10 minute phone consultations are free. Telephone consultations lasting 15, 30 or 50 minutes will be billed $30.00, $60.00, or $120.00 respectively. I can assist with questions about residential, outpatient or inpatient treatment referrals for the above prices.  Telephone consultations with professionals involved in your treatment will be charged in the same manner.   After hours appointments (after 5:00 p.m. or weekends) are available at an increased rate of $140.00 per hour. I expect payment in full at the close of each session (whether you are responsible for the entire fee or you have a co-pay).  I file electronically with all the major insurance carriers.   If I have filed your insurnace correctly and the insurance company fails to pay,  it is your responsilibity to pay me the amount owed by the insurance company.

Fees for St. Francis Weight Loss Surgery Evaluation and the filing of a detailed report are as follows: 

The fee is $175.00 for specialized coaching,  educational materials,  a relapse prevention plan and a detailed report to the physicians at Carolina Surgical Weight Loss. The fee for the psychological testing and evaluation may be covered by insurance so depending on your policy the fee varies.  If you choose to see a therapist at Riley Therapy for follow up visits,  we are in network for Blue Cross Blue Shield,  APS (State Health Plan) and United Behavioral Health (United Healthcare.) It is your responsibility to get preauthorization from your insurance carrier and provide us with the authorization #.  You will also need to find out if you have met your deductible for mental health benefits.  If your plan requires re-authorization of benefits during your treatment,  it is your responsibility to remind us when the new authorization is due.  If you do not and the insurance carrier denies payment to us,  then it is your responsibility to pay us what is owed.   A deposit of $50.00 is required prior to reserving the appointment time.  You can pay this through paypal on the website. The client is expected to pay the remainder of the fee at the time of the evaluation. 


Insurance: 

After the initial educational/coaching/treatment planning session,  some insurance companies will pay a portion of your entire bill. This varies greatly from plan to plan. You must check your policy or contact your company if you wish to know if your plan covers my services. If you are under managed care, you must first call the company for referral or pre-authorization. If your insurance company only authorizes a certain number of sessions,   it is the client's responsibility to remind the therapist to apply for more sessions.  If the client fails to remind the therapist about applying for more sessions,  the patient is reponsible for the amount owed the therapist for any denied visits. If I am not on your panel of providers, you may still have out of network benefits. I can provide you with the necessary paperwork to file with your insurance.   I am in network with Aetna, APS, Blue Cross Blue Shield, Cigna, Medicare, Tricare and United Behavioral Health.

Benefits and Consequences of Psychotherapy:

Persons contemplating counseling should realize that clients frequently make significant changes in their lives. People often modify their emotions, attitudes, beliefs and behaviors. Clients may make changes in their marriage, jobs, or other significant aspects of their lives. Because of counseling, clients may begin to feel differently about themselves and alter significant aspects of their lives. It is important to understand that some of the changes you may be seeking can take time. If you are seeing me for an Eating Disorder, it is not uncommon to have to be seen for three to five years for effective reduction, and/or elimination of symptoms and relapse prevention. If you have questions about the benefits and consequences of counseling, please feel free to ask.

In compliance with state law, you are advised that psychotherapy never includes sexual contact between the therapist and the client or client’s family member. Other dual relationships are prohibited. You are provided with contact information for the licensing boards as follows:

Professional Consultation:

I routinely use the services of professional consultation. These consultants are psychiatrists, psychotherapeutic colleagues, doctors, registered dieticians, and exercise specialists who play an important role in effective intervention. Each consultant is bound by strict confidentiality, and often it is not necessary to reveal client names. If consultation is planned for your case, this will be discussed with you in advance and your consent will be obtained. Please be informed that there are some cases in which failure to provide consent may result in my referring you into the care of another professional, as I may not be able to provide the appropriate care without consultation.


Referrals to Other Professionals:
 

There are times when a referral to a psychiatrist, other medical specialist, registered dietician or exercise specialist is necessary for me to be able to provide the appropriate treatment for the client. If the client chooses not to accept the referral against my advice, it is sometimes necessary for me to terminate my work with this client if I deem that the client’s health is at stake or that continuation of treatment with me without the other professional/s will be of no help to the client.

I earned a Bachelor of Arts degree in Psychology from the Richard Stockton College of New Jersey, and a Master of Arts degree in Counseling Psychology from Centenary College. I am currently working on my LPC Internship.

Appointments:

Appointments may be scheduled by speaking directly with me. An appointment typically lasts 45-50 minutes. If you are unable to keep an appointment, please notify me by telephone only by 5:00 pm the day before the said appointment is scheduled. Otherwise, you will be charged full price for the time reserved. If you miss more than two consecutive appointments without notifying me, you will be subject to termination as a client.

Fees:

Fees will be discussed in your first session. Due to the individualized and collaborative approach used in this practice, each client will receive an unique treatment plan that includes one or more of the following services:

$150.00 initial assessment individual with one therapist (a $50.00 security deposit may be required at the time appointment is made).
$85.00 individual therapy (in office or by telephone) 45-50 minutes.

Other possible fees:
$250 initial assessment individual with family therapist and individual therapist.
$100.00 family therapy (in office or by telephone) with one therapist 45-50 minutes.
$200.00 family therapy (in office or by telephone) with individual and family therapist 50 minutes.
$70.00 group therapy for 90 minutes (includes the cost of food).
$50.00 group therapy for 60 minutes (no food included).
$35.00 family support group for 60 minutes or $50.00 per couple.
$100.00 referral and paperwork for inpatient treatment--one time fee.
$10.00 additional for appointments after 6:00 p.m.

10 minute phone consultations are free. Telephone consultations lasting 15, 30 or 50 minutes will be billed $25.00, $50.00, or $90.00 respectively. I can assist with questions about residential, outpatient or inpatient treatment for the above prices as well. Please be prepared to pay the payment in full upon completion of each session.

Insurance:

I am now accepting Blue Cross Blue Shield and United Behavioral Health.

Confidentiality:

This notice describes how information about you may be used and disclosed. Pleaes review it carefully.

Information shared during your sessions will be held in the strictest confidence. All information revealed by you in a session is placed in your confidential counseling file. Use or disclosure of the following protected information does not require your consent or authorization:

Uses and disclosures required by law—files subpoenaed by a judge.

Uses and disclosures about victims of abuse, neglect, or domestic violence—the duties to warn when someone is likely to endanger the lives of themselves or others, child sexual or physical abuse, elder abuse, etc.

Uses and disclosure for judicial and administrative proceedings—in a case where you are claiming malpractice or breech of ethics.

Uses and disclosures for health and oversight activities—correcting records already disclosed.

Uses and disclosures for law enforcement purposes—when you claim mental health issues as a defense in a civil or criminal case.

Uses and disclosures for research purposes—using client information in research; always maintaining confidentiality. 

Uses and disclosures to avert a serious threat to health or safety—like calling Probate Court for a commitment hearing.
